As a family physician practising in Kepong, I see many patients who delay coming in until symptoms become serious

As a doctor practising in Kepong, I understand why we Malaysians tend to tough things out — we’re busy with work, kids, and family responsibilities, and a clinic visit can feel like an inconvenience we can’t afford. But some symptoms are your body’s way of waving a red flag, and waiting it out can sometimes turn a simple problem into a serious one. Here are five signs I tell my patients and their families never to ignore.

1. Fever That Doesn’t Go Away After 3 Days

A short fever is usually your immune system doing its job against a common viral infection. But when fever persists beyond three days, it’s a different story — especially here in Malaysia, where dengue is a real and constant risk all year round. Dengue can look exactly like a normal flu in the first few days, which is exactly why it catches people off guard.

Many patients tell me they’ve had a fever for 4 or 5 days and simply assumed it was “just a viral fever” that would pass. By the time they come in, some already show warning signs like low platelet counts, severe body aches, or a rash — signs that should have prompted a blood test days earlier.

If your fever lasts more than 3 days, come in for a check-up and blood test. Watch for severe abdominal pain, persistent vomiting, bleeding gums, or unusual bruising — these need urgent attention.

2. Chest Pain or Palpitations — Even Mild Ones

Chest discomfort is one symptom I never want you to brush off, even if it feels mild or comes and goes. Not every chest pain is a heart attack, but the only way to know for sure is to get it checked — because cardiac causes like coronary artery disease or arrhythmias can be silent until they’re not.

I often see patients, especially men in their 40s and 50s, describe a “slight tightness” or “breathlessness climbing stairs” they’ve lived with for weeks, assuming it’s just fatigue or acid reflux.

If you experience chest pain, pressure, a racing or irregular heartbeat, breathlessness, or unexplained sweating — even if it resolves — please get an ECG and a proper assessment. It’s always better to be told “it’s nothing serious” than to find out too late.

3. Unexplained Weight Loss

Losing weight without trying — without changing diet or exercise — is one of the clearest red flags in medicine. It can be an early sign of thyroid problems, uncontrolled diabetes, underlying infections, or malignancy. If you’re not dieting but the weight keeps dropping, your body is telling you something.

I’ve had patients proudly tell me they’ve “lost weight without even trying” when in fact they’d lost 5–6 kg over two months alongside fatigue and poor appetite they hadn’t connected to it.

Rule of thumb: if you’ve lost more than 5% of your body weight over 6–12 months without a clear reason, get a proper work-up — blood tests and sometimes imaging — to find out why.

4. A Wound or Infection That Isn’t Healing

A small cut or wound that won’t close, or keeps looking red and swollen, deserves attention — particularly if you have diabetes or haven’t been screened for it. Poor wound healing is a hallmark complication of uncontrolled blood sugar, especially in the feet and lower legs.

I’ve treated patients who mention a “small scratch” on the foot that’s been there for three or four weeks, getting worse, while they kept applying the same ointment at home. Left unchecked, these wounds can progress to serious infections.

If a wound hasn’t improved within one to two weeks, or looks infected, have it examined. It’s also a good opportunity to check your blood sugar — a slow-healing wound may be your first clue.

5. Persistent Fatigue Lasting More Than 2 Weeks

We’re all tired sometimes — work deadlines, young children, traffic on the way to Kepong during peak hour. But fatigue that lingers more than two weeks, doesn’t improve with rest, and affects daily life is different. It can point to anaemia, thyroid dysfunction, undiagnosed diabetes, or other conditions a simple blood test can uncover.

Many of my patients, particularly working mothers, describe feeling “just tired all the time” for months, assuming it’s the demands of family life. Often, a blood test reveals iron-deficiency anaemia or a thyroid imbalance — very treatable once identified.

If you’ve been unusually drained for more than two weeks despite adequate sleep, don’t just push through it. A basic blood panel can often point us straight to the cause.

When in Doubt, Get It Checked

Your body rarely sends these signals for no reason. Early detection is almost always simpler, cheaper, and safer to treat than a problem that’s had months to grow. As a family physician, I’d always rather reassure you that everything is fine than see you again later with something that could have been caught earlier.

At Klinik Medilove Keponggi, we see families across Kepong for exactly these concerns — from a fever that won’t quit to a wound that isn’t healing.
